Northern Plains Healthy Start Community Coordinator Job Description

POSITION TITLE: Community Coordinator

This position plans and implements strategies designed to fulfill the established Northern Plains Healthy Start program goals and objectives for the NPHS site, and is responsible for the overall management, development, and accountability of the program at the site, as well as Case Management activities. The Tribal Health Director or designee will provide direct day-to-day supervision in collaboration with the NPHS Director.

SPECIFIC DUTIES:
Formulate a course of action that will assist in the implementation of project objectives.

Ensure that community activities are in compliance with all AATCHB and NPHS policies, guidelines and procedures.

Responsible for the daily management of the local NPHS project, in accordance with
all contractual and cooperative agreements.

Responsible for the collection of project data and writing monthly reports to project Director, with copy to the Tribal Health Director.

Responsible for the local financial management of the project, in accordance with federal policies and all other applicable procedures. Works with the Finance Officer to maintain accounting of program funds.

Responsible for the management of site staff development.

Provide clear and concise recommendations to the Tribal Health Director, staff and community members of policy changes and proposed programs affecting infants and families.

Coordinate and assist tribal officials in the development of a sustainability plan to keep Healthy Start operational.

Responsible to perform case management duties at a minimum of 25% time in accordance with the standards of the project’s Targeted Case Management Model.

Advocate for and participates in local inter-disciplinary Infant Mortality Review activities.

Promote, through collaboration with local agencies, public awareness and education, on the importance of Maternal Child Health issues and the risk factors associated with infant mortality.

 

QUALIFICATIONS:

Must be knowledgeable in the area of Maternal Child Health issues.

Must be able to present, communicate and interact with both public and private health officials at tribal and community level concerning program issues.

Possess leadership and organizational skills.

Must be trainable in the area of NPHS data and statistical collection tools.

Must be knowledgeable of the Indian Health Service operations.
